Managements of Craniophayngioma at Primary and Recurrent Cases

Abstract
Objective: The objective of this study was to review retrospectively the 25-year experience of craniopharyngiomas for the primary and recurrent cases considering recurrence rate, progression-free survival(PFS) time, and visual function.

Methods: From 1981 to 2005, 65 patients were operated in our hospital. Among them, 36 patients followed up over 36 months were surveyed. The median age was 22 years(range, 4-80 years) and the median follow-up period was 79.5 months (range, 36-280 months).

Results: In the primary treatment, gross-total resection(GTR), subtotal resection(STR), and STR with adjuvant radiotherapy(XRT) within 2 months were achieved in 11(31%), 11(31%), and 14(38%) patients respectively. 3 patients(27%) with GTR showed the recurrence with the longest PFS time(median 136 months). 18 patients with recurrent tumors were performed with treatments of total 44 times(GTR, STR, STR with XRT, radiosurgery, or omaya reservoir). STR with XRT showed the lowest recurrence rate(31%) with the longest PFS time(168 months) in the recurrent tumors. 17 (60.7%) patients showed the improvement of visual function and visual function was related to treatment times inversely.

Conclusion: This study provides that GTR was the most effective treatment in primary craniopharyngioma and STR with XRT in recurrent tumors. Visual function can be improved with the appropriate treatments.
